The blood transfusion puzzle in Silent Hill: Townfall is more difficult than scary, and that's saying something, as this challenge takes place in a dark and monster-ridden hospital. To solve the blood transfusion puzzle, you must find the missing blood bags, place the blood bags in the correct slots, and find a connection between blood types.

Here's where to find the blood bags and how to complete the blood transfusion door puzzle in Silent Hill: Townfall's "Many Into One" mission.

During the "Many Into One" mission in Silent Hill: Townfall, which follows the patient puzzle, you will stumble upon a large, locked door. Grab the map from the counter in front of the door, and you'll find that it's called the blood transfusion machine. The machine draws blood from the blood bags attached to the walls on your left and right.

There are three empty blood bag slots; if you fill them up, you will solve the blood transfusion puzzle and the door will open. To complete the full blood transfusion door puzzle, you must complete the following steps:

Find the missing blood bags.
Solve the blood bag puzzle by placing the bags in the correct slots.
Solve the blood transfusion door puzzle by connecting the correct blood types.

Find the missing blood bags in Silent Hill: Townfall

Take the eastern corridor and enter the last room to the right. Crawl through the vent in the southwest corner (location 1 on the map below) to find Alan Duncan's blood bag (location 2). Be sure to grab Zoe's patient list and play the recording before leaving this room. Attach Alan Duncan's blood bag to the door in the west corridor (location 3). Now you can enter the west wing.

If you took Zoe's patient list in Alan Duncan's room, the locations of the next two blood bags are marked on your map: Sarah Watson's blood bag is in room 05 (southwest), while Lesley Greer's blood bag is in room 12 (northeast). As room 12 is inaccessible for now, pass through rooms 3 and 4 to avoid the monster, grab Sarah Watson's blood bag in room 5 (location 1 on the map below), and head down the staircase in the west.

On the lower level, sneak past the monsters into room 4 (location 2), where you must collect the Ward W Master Key. Be careful; you can't kill the monsters, and you will die instantly if they grab you. This key can be used to unlock room 13 (location 3), where you must take the ladder upward. From there, you can enter room 12 and grab Lesley Greer's blood bag (location 4).

Return to the area with the blood transfusion machine and use Lesley Greer's blood bag to enter the east wing. Distract the monster to sneak into room 3 (location 1 on the map below), from whence you can enter room 2. Matthew Clark's blood bag is attached to the corpse in the elevator (location 2), but before you can grab it, it'll drop to the lower floor.

Don't go downstairs yet! Instead, crawl through the vent between the staircase and room 6 (location 3). Drop down into room 10, where you'll find George McEwan's blood bag and the Ward P Master Key (location 4). Use the key to open room 3 on the same floor level and collect Matthew Clark's blood bag (location 5).

Now that you've got every blood bag, head back to the blood transfusion machine upstairs. As the nearest exits are locked, crawl through the vent in room 2 to escape (location 1 on the map below). Upstairs, turn the wheels in rooms 5 and 10 (locations 2 and 3 on the map below). This will open the door back to the transfusion puzzle. Beware of the unkillable monster!

How to solve the blood bag puzzle in Silent Hill: Townfall

To match the blood bags to the right transfusion slot, you must match their description (hover over them in your inventory) to the patient info left at each slot. So, to open the blood transfusion machine, place the blood bags in the following spots:

How to solve the blood transfusion door puzzle in Silent Hill: Townfall

After the first door opens, you'll find a second transfusion door puzzle. This time, you must connect the center of the circle to three of the eight blood bags. The most important clue is the name at the center of the circle: Ordell. This is Simon's last name (Silent Hill: Townfall's protagonist).

Open your inventory, and you'll find Simon's medical wristband, informing you that his blood type is A+. See the plaques to your left and right? They reveal which blood types can donate to each other. Blood type A+ is connected to blood types O-, O+, A-, and A+.

By checking the blood bags on the walls, you'll find that Thomas Fergusson, Hannah McGregor, and Sarah Watson match Simon Ordell's blood type. The solution to the blood transfusion door puzzle is to connect "Ordell" to the initials T.F, H.M, and S.W.
